Annual Health and Medical Record
Each Scouting Adventure, camping trip, high-adventure excursion, and having fun
are important to everyone in Scouting. So is YOUR safety, and the well-being of YOUR Scout.
For that reason, personal Medical History forms are kept at the Troop level and used when needed.
Completing the Annual Health and Medical Record is the first step in making sure you have a great Scouting experience.
Forms A and B are for Any and ALL Events --- ADD - Form C for Summer Camp --- ADD - Form D for High Adventure

Follow this link, to complete the forms for your son, yourself, and any siblings that may attend, any and ALL Boy Scout functions.
REMEMBER,
This is an annual form... to be completed every year,
and we will need a copy of the Parent/Guardian Health Insurance card for our Records.
FOR DRIVERS OF ANY SCOUTS TO AN EVENT
The Troop will need a copy of your Vehicle Insurance Card
